Background    
IL-6Rα (CD126), a subunit of IL-6 receptor complex (shared by other cytokines), belongs to type I cytokine receptor family. It has 468 amino acids, with N-terminal fibronectin type-III domains (113-316 a.a.) and exists as membrane-bound (full-length) or soluble (1-365 a.a., from proteolytic cleavage) isoforms. It binds IL-6 with low affinity; signal transduction requires association with gp130 (IL6ST), activating JAK/STAT, MAPK, Akt pathways. It regulates immune responses, cell growth/differentiation, acute-phase reactions, and hematopoiesis. Dysregulation links to autoimmune diseases, multiple myeloma, and inflammatory malignancies, with sIL-6R acting as an IL-6 agonist via trans signaling.
